--Heavy dust build up on walk in produce cooler refrigeration unit fan guards.
--2 overhead doors located in receiving area of the food facility have gaps at bottom of doors and do not protect against entry of insects, rodents.
--Approximately 25 old rodent pellets observed on pallets in storeroom area around sugar.

--No sign or poster posted at the handwash sink in the restroom area to remind food employees to wash their hands.
--Rear emergency door located in the storeroom area of the food facility has a gap and does not protect against the entry of insects, rodents.

--Food facility has an original certificate posted, but the location is not conspicuous for public viewing.
--Toilet rooms do not have a self-closing door.
--Observed wet wiping cloths in front prep area, not being stored in sanitizer solution.
--Dumpster lids are closed attracting birds.
--Food employee observed making change with money and then proceeded to make pizza without washing hands.
--Paint peeling on rear walk in cooler wall and ceiling surfaces
--Old greasy food residuals observed on interior surfaces of the ware wash sink sanitizing compartment.
--Food facility has lost its certified food employee over 3 months ago and has not replaced the certified food employee as required.
--Soap was not available at the handwash sink in the front prep area.

--Containerized food stored directly on the floor in the walk-in cooler, rather than 6 inches off of the floor as required. The box was elevated during this inspection.
--Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was <10 ppm, and not 50-100 ppm as required. it was determined that the sanitizer dispensing tubing was clogged with a white residue. the repairman came and serviced the machine and the residual sanitizer is now 50 ppm.

--Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration
--Prepackaged baked goods and re-packaged food items are not labeled to clearly indicate any "Big 8" allergen ingredients and / or the allergen warning statement.
--Prepackaged Baked goods ie. cinnamon rolls, cupcakes, country Italian bread, cinnamon apple walnut bread, dinner rolls candy cookies, as well as repackaged pudding, potato and chicken salad, and Cole slaw are not labeled properly with an ingredient statement. There are no placards with ingredient statement either.
--The wall in the back and left hand side in the storage area have several holes in the drywall and in need of repair. There is a hole in the wall behind the door in the ladies restroom and in need of repair. The PIC stated that the owner of the building is to make these repairs but she is not sure when they are supposed to be doing it.
--There are approximately 63 floor tiles that are missing or broken throughout the storage room and floor is no longer smooth and easily cleanable. The PIC stated that there is a person who was to come repair the floor in July but called and said he is back logged but will come and repair floor tiles.
--There are 11 floor tiles that are broken/missing in front of the reach-in McCray freezer in the retail area of the facility.

--Observed single use Soy Sauce buckets being reused to store other food items such as onions and bean sprouts inside the walk-in cooler. These food items were moved to clean food safe grade buckets intended for re-use.
--Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in the Bain Marie.
--The bain marie is not holding food items at a temperature of 41*F or below. The Person in Charge stated that they will call repairman today to come and repair the unit. The facility will not use this bain marie to store Temperature control for safety foods until it is holding temperature at 41* F or below.
--The post can opener blade, a food contact surface, was observed to have food residue and was not clean to sight and touch. The can opener was cleaned and sanitized during this inspection.
--Observed an in-use knife and cleavers stored between the stainless table and the side of the Bain Marie, an area not easily cleanable & sanitized. The knife and cleaver were removes, cleaned and sanitized during this inspection.
--A common cup/bowl (without a handle) being used as a dispensing utensil was observed stored in 2 bulk ingredient containers. The bowls were discarded during this inspection.
--Observed containerized food stored directly on the floor in walk-in cooler rather than 6 inches off of the floor as required. The containers were elevated during this inspection. The containers were elevated during this inspection.

--Food staff observed in kitchen area, wearing watch on arm.
--2 hoses observed laying on ground along rear of kitchen that are connected directly to water line without benefit of a back flow preventative device(vacuum breaker).
