250 S Church ST Hazleton, PA 18201
OUT OF COMPLIANCE
--Facility does not have written procedures for employees to follow when responding to an event involving vomitus or fecal matter discharge onto surfaces within the facility.
--Assorted knives stored in storage box at prep line have old dried food residuals on blades.
--Grey water is being disposed of onto paved lot behind establishment as evidenced by staining / grease on pavement.
--Person(s) in charger are not monitoring establishment operation for compliance with food code requirements.
--Food Employee observed donning single use gloves without a prior hand wash.
--Food prep area is in need of a thorough cleaning.
--Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers.
--Approx. 20 gallons of cooked marinara sauce stored in plastic buckets in walk in cooler are 82*f product center. Items were made on previous day as stated by person in charge. All product was voluntarily disposed.
--Marinara sauce is stored in 5 gallon plastic buckets to a depth that does not allow for proper cooling.
--No consumer advisory posted on menus for animal derived foods cooked to order.
--Pans of cooked bacon are stored on moldy cardboard used as shelf liner in walk in cooler.
--Paper towel dispenser empty at the hand wash sink in the food prep area.
--Chemical spray bottle on rack in kitchen is not labeled for content.
--Observed 2 open pots of potatoes cooling outside in lot behind building.
--Floor tiles deteriorated / missing at cook line.
DONUT CONNECTION

--Food employees observed in food prep area, not wearing proper hair restraints, such as nets or hats.
--3 basin sink has build up of grime in basins.

KING'S DEN

--3 basin bar sink has build up of mineral deposits.
HAZLETON PALACE BUFFET
1057 N CHURCH ST Hazle Township, PA 18202
--2 dead roach casings are observed in tray holding cooler in pantry behind hibachi grill.
--Several holes observed in lower wall coverings in several spots in kitchen.
--Floor to wall junctions have build up of spilled food residuals / grease / grime.
--Non-food contact surfaces underneath hibachi grill are not cleaned at a frequency to preclude accumulation of dirt, soil and grease. Lower cabinet spaces below some buffet tables are not being cleaned frequently enough to prevent build up of spilled food residuals.
