The post can opener blade, a food contact surface, was observed to have dried black food residue and was not clean to sight and touch. The post can opener was cleaned and sanitized during this inspection

Observed containerized food stored directly on the floor in walk-in freezer area, rather than 6 inches off of the floor as required. The boxes were elevated during this inspection.

Observed raw eggs and meat stored abovr RTE foods in the refrigerator. Eggs and meat were moved.
Observed clean food equipment in kitchen area, stored uncovered or not inverted.

Observed a build up of old food residue on the exterior of the storage cabinet in the walk in freezer.
Observed a build up of dust on the fan guard in the walk in freezer.
Observed a build up of grease around the shaft on the mixer.

Observed containers with no handles being used as scoops and stored in bulk ingredients. Containers were removed.
Deteriorating rubber door gasket observed on the one cooling unit.
Observed a build up of food residue on the interior of the microwave.
Observed old food residue on the post can opener blade.
Observed a build up of rust on shelves in the walk in cooler.
Observed deeply scored cutting boards not resurfaced or discarded as required.
Observed build up of dust on the fan guard in the pizza Bain Marie.
Observed a build up of dust on the equipment storage shelving.

Observed a build up of food residue on the handle of 1 freezer unit.
Observed deeply scored cutting boards not resurfaced or discarded as required.
Temperature measuring device for ensuring proper temperature of equipment is not available in drawer unit.

Food employees observed in prep area, not wearing proper hair restraints, such as nets or hats. Hat was provided.

Observed a build up of grease on the surface around the paddle on the small mixer.
Observed clean stored in back area directly on the floor, and not 6 inches above the floor. Itms were cleand and sanitized.
Observed dead rodent on clue trap under equipment. Rodent was discarded.
Multiple food ingredient storage containers, in the prep area, not labeled with the common name of the food.
Observed buckets of ingredients stored directly on the floor in walk in cooler, rather than 6 inches off of the floor as required.
Observed too numerous to count rodent like droppings under equipment and along the wall in the back area.
Observed the coating peeling on some shelves in the walk in cooler.

THE AVENUE RESTAURANT AND CATERING

Food employees observed in prep area, not wearing beard covers.
Exit door located in the rear kitchen area of the food facility has a gap at the floor door junction and does not protect against the entry of insects, rodents, and other animals.
Flooring in the prep and dish washing area is made of tile and is cracked or missing tiles/ roughened / and is not a smooth, easily cleanable surface. The owner stated he has contracted a with a construction company to replace flooring. He is waiting for the contractor to let him know exactly what dates he is going to be coming in after closing to replace the flooring.
